3.3.6.2 Return From LOA

POLICY

Notification of intent to return requirements are available in the appropriate contract and Personnel Manual for All Non represented Personnel. If leave was health-related, an employee may not return to work until a Health Questionnaire, 10-2300, or a Physicians Report on Illness, 10-2305, (whichever is required or deemed advisable) has been submitted and approved.

PROCEDURE

For ACADEMIC employees:

  1. Notify Employing Unit of intent to return to work.

Employing Unit

  1. Prepare Personnel Assignment (PASS), 10-787.

  2. Send PASS to Provost.

  3. Prepare First/Last Day Notice, 10-618, and distribute copies to appropriate departments.

Provost

  1. Review and sign PASS to indicate approval.

  2. Send to Human Resources.

For NON-ACADEMIC employees:

  1. Notify Employing Unit and Human Resources of intent to return to work.

Human Resources

  1. Prepare PASS.

  2. Send Personnel Assignment (PASS), 10-787, to Employing Unit.

Employing Unit

  1. Obtain authorizing signatures on PASS.

  2. Return to Human Resources.

  3. Prepare First/Last Day Notice, 10-618, and distribute copies to appropriate departments.

For ALL employees:

  1. Verify positive attributes and values.

  2. Return PASS to Employing Unit if inconsistent or incorrect.

  3. Enter into HRS.

  4. Send copies of PASS to Payroll and Benefits Administration.

  5. Distribute copies of PASS.

  6. Send Notice of Personnel Action (NPA), 10-786, to employee.
