09-1 Mandatory Immunization Records (Third Release)

  1. Purpose
    1. It is the policy and practice of Wayne State University to protect the public health of the WSU community. The recommendations in this Policy are intended to protect the public health of the WSU community.
    2. After reviewing the recommendations of the United States Centers for Disease Control, the Advisory Committee on Immunization Practices, and the American College Health Association guidelines, it is recommended that all WSU students have current age appropriate immunizations. Those age appropriate immunizations are set forth at the following websites:

      For students 19 and older: https://www.cdc.gov/vaccines/schedules/easy-to-read/adult-easyread.html.

      For students up to age 18: https://www.cdc.gov/vaccines/schedules/easy-to-read/adolescent-easyread.html.
    3. This University Policy outlines the expectations for reporting immunization records by all enrolled students at Wayne State University.
  2. Definition
    1. Personal Immunization Record: ‚ A document, originating from a state immunization registry or an official medical record signed by a licensed health care provider, listing which recommended immunizations the student has or has not received.
  3. Policy and Procedure
    1. It is strongly recommended that all WSU students in the campus community have their Personal Immunization Record on file at the Campus Health Center.
    2. In accordance with the Health Insurance Portability and Accountability Act of 1996 (HIPAA), these records of personal health information will be maintained at the Campus Health Center as part of the student's personal health record.
    3. Through its independent relationship with public health authorities, the Campus Health Center has access to the Michigan Care Improvement Registry (MCIR) which contains immunization records of individuals born after 1994 who are in the registry. The Campus Health Center also partners with local public health authorities for public health concerns related to vaccine preventable communicable disease cases.
    4. All students not born in the State of Michigan will not be in MCIR and are directed to provide their Personal Immunization Record to the Campus Health Center.‚  Any student who is unaware of their immunization status is strongly encouraged to visit the Campus Health Center to ensure immunizations are up to date.
    5. All appropriate offices on campus including the Dean of Students, the Campus Health Center, the Office of International Programs, and Housing and Residential Life will communicate this recommendation to students annually along with the procedure for providing the Personal Immunization Record.
    6. Nothing in this policy is intended to replace the immunization and reporting policies of individual schools, colleges and divisions within the university who have specific requirements related to academic or work requirements. Students and employees should continue to adhere to any such requirements in addition to this Policy.
  4. Duration and Effective Date
    1. This University Policy may be modified or revoked by the President at any time and without notice. ‚ 
    2. This university policy is effective upon issuance.
