Before Submitting an Administrative Systems Access Request Form

Instructions: Contact the Data Custodian Representative(s) indicated below to request authorization to access the Alumni/Development Information Systems (ADIS) Database, any portion(s) of the Student Information System (SIS), or the entire SIS/FOCUS Database.  To indicate approval of the authorization request, the Data Custodian Representative(s) can either sign the request form or send written approval in an electronic mail message.
