1.0  Purpose

1.1     It is the policy and practice of Wayne State University to protect the public health in the University residence halls.

1.2     After reviewing the recommendations of the United States Centers for Disease Control and the Advisory Committee on Immunization Practices, it is recommended that all students residing in residence halls have current vaccinations against Hepatitis B, Measles, Mumps, Rubella, Varicella, Meningitis, and Tetanus, Diptheria, and Pertussis.   

1.3     This University Policy outlines the procedures for implementation of this policy at Wayne State University.

2.0  Definition

2.1     Personal Immunization Record:  A document listing which recommended immunizations student has or has not received, completed and signed by licensed health care provider.  

3.0  Policy and Procedure

3.1     Wayne State University shall require each student taking up residence in a University residence hall to provide a completed personal immunization record indicating which if any immunizations a student has received, prior to occupancy.

3.2     In accordance with the Health Insurance Portability and Protection Act (HIPPA), these records of personal health information will be maintained at the Campus Health Center as part of the student’s personal health record.

3.3     Wayne State University Office of Housing and Residential Life and the Campus Health Center will coordinate and collaborate in the collection of the personal immunization record for each student.  

3.4     The Campus Health Center will provide the Office of Housing and Residential Life a list of students submitting completed immunization records, but will not release any information related to immunization status of any student.

3.5     Compliance with this policy will be enforced by a hold on student’s access to occupancy within the residence hall until completed immunization record has been submitted to the Campus Health Center.  

4.0  Duration and Effective Date

4.1     This University Policy is revocable by the President at any time and without notice.
