3.3.6.1 Assignment To LOA

POLICY

Eligibility for continued benefits is dependent upon the union contract and/or the Personnel Manual for All Non represented Personnel in effect when the leave begins.

PROCEDURE

For LONG-TERM DISABILITY Leaves:

Responsibility

Action

Benefits Administration

  1. Receive determination of the start date from insurance company.

  2. Inform Employing Unit and Human Resources of approved Leave.

Employing Unit

  1. Prepare First/Last Day Notice, 10-618, and distribute copies to appropriate departments.

  2. Initiate Personnel Assignment(PASS), 10-787.

  3. Send PASS to Human Resources.

For all other leaves for NON-ACADEMIC employees:

  1. Determine the effective dates.

  2. Inform Employing Unit to prepare PASS.

Employing Unit

  1. Prepare PASS.

  2. Send PASS to Dean/Director.

  3. Prepare 10-618 and distribute copies to appropriate departments.

Dean/Director

  1. Review and sign Personnel Assignment (PASS), 10-787, to indicate approval.

  2. Send to Human Resources

For all other leaves for ACADEMIC employees:

  1. Acquire approval from Provost for Leave.

  2. Prepare PASS.

  3. Send both to Dean/Director.

  4. Prepare First/Last Day Notice, 10-618, and distribute copies to appropriate departments.

  1. Review and sign PASS to indicate approval.

  2. Send PASS and memo to Provost.

Provost

  1. Review and sign PASS to indicate approval.

  2. Send PASS to Human Resources

For ALL leaves:

  1. Enter leave into HRS.

  2. Send copies of PASS to Payroll and Benefits Administration.

  3. Distribute copies of PASS.

  4. Send Notice of Personnel Action (NPA), 10-786, to employee.
