POLICY
Eligibility for continued benefits is dependent upon the union contract and/or the Personnel Manual for All Non represented Personnel in effect when the leave begins.
PROCEDURE
For LONG-TERM DISABILITY Leaves:
Responsibility | Action |
Benefits Administration | -
Receive determination of the start date from insurance company. -
Inform Employing Unit and Human Resources of approved Leave. |
Employing Unit | -
Prepare First/Last Day Notice, 10-618, and distribute copies to appropriate departments. -
Initiate Personnel Assignment(PASS), 10-787. -
Send PASS to Human Resources. |
For all other leaves for NON-ACADEMIC employees:

Determine the effective dates. -
Inform Employing Unit to prepare PASS. |
Employing Unit | -
Prepare PASS. -
Send PASS to Dean/Director. -
Prepare 10-618 and distribute copies to appropriate departments. |
Dean/Director | -
Review and sign Personnel Assignment (PASS), 10-787, to indicate approval. -
Send to Human Resources |
For all other leaves for ACADEMIC employees:

Acquire approval from Provost for Leave. -
Prepare PASS. -
Send both to Dean/Director. -
Prepare First/Last Day Notice, 10-618, and distribute copies to appropriate departments. |
